New COVID-19 Information for your next visit

 

DO NOT COME TO THE OFFICE IF YOU FEEL UNWELL!

If you feel unwell, please call. me at 519-979-2154 to postpone your appointment. 

**Instructions For Clients**

 

1. 24 hours before your appointment, please Take the Ontario COVID-19 Online Assessment. 

 

2. The building is locked. Please wait in or around your car until I call you for your over the phone COVID-19 screening (or I can come out to your car for screening if you don't have a cell phone). On your appointment day, please arrive as close to your appointment time as possible. 

 

*If you screen COVID-19 positive, your appointment will be postponed and you will not be permitted into the buidling. Note, this does NOT mean you have COVID-19. 

 

If you screen positive, you cannot be treated for at least 14 days, be asymptomatic, AND you must be cleared by your primary care provider or Telehealth Ontario of any concern of COVID-19.

 

3. Only pre-screened clients will be allowed to enter the building. If you require assistance or are bringing a minor for an appointment, please advise me in advance.  

 

4. Please leave extra bags and belongings in your car or at home or work.  You can bring your treatment clothes in a small bag or wear them to your appointment.

 

5. Be prepared to pay with credit or debit card. Please do not pay with cash if possible. 

 

6. All clients will use the back door entrance to the building only. Upon entering the building, you will be asked to wash your hands and then don your clean mask if you have brought one (see #7). Hand sanitizer will be provided, or you can use soap and water in the washroom.

 

7. Clients are asked to bring their own clean mask if they have one (or I can provide one if you do not have one). It is strongly encouraged by our governing body that you wear a mask during your entire visit if you can tolerate wearing one. If you feel you cannot tolerate wearing a mask, please call me at least 24 hours before your appointment, so I can assess risk to provide treatment.
